Serving over 100,000 visits annually, CSC provides a wide range of services through its Social Services, Federally Qualified Community Health Center, Youth Center, and Community Economic Development programs. The organization offers linguistically and culturally responsive support in multiple Chinese dialects, along with Vietnamese, Spanish, and more.
With an annual operating budget exceeding $10 million, CSC is funded by federal, state, and local governments, private foundations, and community donations, all contributing to programs that foster self‑sufficiency and meet the evolving needs of the community.
This is a contract, full‑time on‑site role for a Case Manager located in Monterey Park, CA. The Case Manager will provide direct services to clients by conducting needs assessments, creating service plans, and connecting clients with appropriate resources, including social services, financial assistance programs, and healthcare providers. Responsibilities include case documentation, follow‑ups to ensure service effectiveness, and collaborating with community partners to advocate for clients.
The Case Manager will also assist in community outreach efforts, promote program participation, and ensure compliance with organizational policies and funding requirements.
- Strong interpersonal skills, including empathy, active listening, and cultural competency when working with diverse communities.
- Experience in case management, conducting client assessments, and developing personalized service plans.
- Knowledge of social services programs, eligibility requirements, and resource referral networks.
- Organizational and time‑management skills, with the ability to maintain detailed documentation and manage multiple cases.
- Proficiency in English and fluency in one or more of the following languages:
Cantonese, Mandarin, Toishan, Chiu Jou, Vietnamese, or Spanish. - Bachelor’s degree in Social Work, Psychology, or a related field preferred; equivalent experience may be considered.
