Location: Châteauguay
Un centre de santé et de services sociaux dans la région de Châteauguay recherche un(e) Infirmier·ère pour assurer le suivi post-hospitalisation. Vous serez responsable de réduire les réadmissions, de contrôler les soins post-hospitalisation, et d'assurer le suivi des symptômes et de la médication des usagers. Un diplôme d’études collégiales en soins infirmiers ou un baccalauréat est requis, ainsi que l’adhésion à l'OIIQ.
Une rémunération compétitive et quatre semaines de vacances après un an sont offertes.
