Unicity Care Management is seeking a results‑driven Clinical Liaison who is passionate about selling care management solutions that help individuals and their families make safer, smarter care decisions. In this role, you will blend clinical insight with consultative sales skills to identify needs, present Unicity’s value, and convert inquiries and referrals into ongoing care management engagements.
This position is ideal for an RN, LPN, social worker, or experienced care professional who enjoys being in the community, building referral networks, and guiding families toward the right care management services.
Key Responsibilities- Grow care management revenue by generating, qualifying, and converting referrals from hospitals, rehabs, SNFs, assisted living and memory care, physician practices, attorneys, financial advisors, and community agencies.
- Position and “sell” Unicity’s care management programs by clearly explaining assessments, care planning, coordination, advocacy, crisis support, and ongoing monitoring as solutions to client and family pain points.
- Conduct structured pre‑engagement consultations (phone, virtual, or in person) to uncover needs, handle objections, review scope and pricing, and secure commitment to begin care management services.
- Respond rapidly to new inquiries, gather clinical and psychosocial information, and recommend the most appropriate care management service options.
- Partner closely with Care Managers to ensure a smooth transition from sales to service delivery, including warm handoffs for assessments, care plans, and follow‑up.
- Represent Unicity at professional meetings, discharge rounds, community education events, resource fairs, and networking functions to increase market awareness and generate new leads for care management.
- Maintain ongoing contact with key referral partners and families to nurture relationships, monitor satisfaction, and identify new opportunities for additional or expanded care management support.
- Meet or exceed defined targets for outreach volume, consultations held, new care management starts, and account development activity.
- Active RN, LPN, social work license, or other relevant clinical/behavioral health background strongly preferred.
- Prior experience in a Clinical Liaison, Care Management Liaison, Community Relations, or similar outreach/business development role is highly desirable.
- Strong understanding of complex medical and psychosocial needs, chronic illness, family dynamics, and community resources, with the ability to translate this knowledge into compelling recommendations.
- Ability to interpret medical records and discharge plans and frame them as opportunities for care management interventions that improve safety, outcomes, and family peace of mind.
- Excellent communication and presentation skills with both professionals and laypersons; comfortable having complex conversations about care, safety, and next steps.
- Organized, self‑directed, and motivated by clear outreach and growth goals in a mission‑driven setting.
- Valid driver’s license, reliable transportation, and ability to travel routinely within the assigned territory.
