Overview

The Case Manager – Housing conducts comprehensive, housing-focused assessments and delivers ongoing case management services to individuals and families experiencing housing instability. This position supports participants in securing and maintaining safe, stable housing; developing independent living skills; increasing income and financial stability; and building long-term self-sufficiency. The Case Manager – Housing works within a Housing First framework and collaborates closely with community partners to reduce barriers and connect clients with needed resources.

This position is supervised by the Human Services Manager – Economic Support. This is a part-time unbenefited position.

Essential Duties And Responsibilities
  • Perform a personalized screening that identifies the member’s preferences for, and barriers to, housing, including accessibility and affordability.
  • Identify other needs related to accessing Medicaid and non-Medicaid services (social, educational, or other services, regardless of funding source), including referrals to these services and supports.
  • Collaboratively develop a person-centered plan based on the identified needs. The supportive housing agency cannot bill for consultation services until the person-centered plan is complete.
  • Create a person-centered plan for transition, sustaining, and relocation housing support services that meet the member’s needs as identified during their screening. Members will have freedom to choose which supportive housing agency they work with for transition, sustaining, and relocation services.
  • Identify other needed services and supports, and support access to these needed services through proper referrals.
  • Assist clients in completing housing applications, including all documents necessary for a successful application.
  • Support move-in to new units, inspections, and develop plans for rent payment.
  • Provide case management to participants after housing placement, supporting them in adjusting to housing and building long-term connections to community services.
  • Recruit landlords to accept subsidized housing clients, work closely with housing authority to secure leases, and maintain ongoing relationships with landlords to support client housing stability.
  • Verify housing units are safe, ready for move-in, and meet Habitability Standards.
  • Provide education on the role, rights, and responsibilities of the tenant and landlord.
  • Identify what kind of early prevention measures or interventions could address behaviors or occurrences that put housing at risk (for example, lease violation or hospitalization) and implement those interventions if housing is in jeopardy.
  • Tacking clients enrolled in rapid re-housing.
  • Link participants with services for mental/physical health, housing, substance recovery, and other services based on initial intakes and assessments.
  • Help identify potential housing opportunities, complete paperwork, and prepare for transitional housing.
  • Utilize motivational interviewing techniques. Explore participants’ substance abuse using Harm Reduction Model.
  • Compile data, prepare reports, and maintain thorough and concise case notes.
  • Participate in program and case meetings.
  • Participate in Southwest Coalition Coordinated Entry Case Conferencing.
  • Monitor and review participant progress toward meeting service or therapeutic goals and objectives and for housing outcomes in service plans.
  • Compile and maintain required documentation.
  • Communicate orally and in writing with participants, appropriate family, guardians, service providers, county/state administration and interested members of the community.
  • Provide assistance to participants to access necessary medical care and treatment.
  • Provide assistance to participants as appropriate, to pursue vocational and/or educational opportunities.
  • Participate in the semi-annual Point in Time Homeless Count.
  • Other duties as assigned.
