Geriatric Physician:

We are seeking a Board-Certified Physician to join our established geriatric medicine team serving skilled nursing facilities. This position offers complete control over your weekly schedule, predictable hours, and the opportunity to provide high-quality elder care without the typical constraints of traditional medical practice.
Location: Monroe, WI (serving southwestern Wisconsin and northern Illinois communities)
Schedule: Monday through Friday only - YOU set your specific days and hours
Patient Panel: Established census - begin seeing patients immediately
- Set your own working hours each week
- Choose which days you work
- Decide your start and end times daily
- Adjust your week-to-week schedule based on personal needs
- Personal scheduler assigned exclusively to coordinate your preferences
- They handle all facility coordination and appointment logistics
- Zero on-call responsibilities
- No weekend coverage requirements
- No evening or holiday obligations
- Predictable Monday-Friday framework you customize
- Provide comprehensive medical care to residents in skilled nursing facilities across the Monroe area.
- Conduct routine assessments, medication management, and care plan development
- Respond to acute changes in patient condition during your scheduled hours
- Supervise and collaborate with a team of experienced Geriatric Nurse Practitioners
- Complete documentation in the EPIC EMR system
- Participate in interdisciplinary care team meetings
- Communicate with patients, families, and facility staff regarding treatment plans
- Established patient panel in long-term care and post-acute settings
- Geriatric patients with complex medical needs
- Board Certification in Family Medicine OR Internal Medicine
- Dual Licensing: WI and IL
- Active medical license in Illinois (or ability to obtain prior to start date)
- DEA certification
- Current BLS certification
- Previous experience in post-acute care settings
- Previous experience in long-term care or skilled nursing facilities
