The LPN COORDINATOR will:

  • Participate in data collection related to patient status and needs, as appropriate to the ages of the patients, growth and development through the life cycle (Adults, Geriatrics). Interview patients and gathers identifying data and health history. Observe and report emergency information to the Physician immediately and on an ongoing basis throughout the visit. Document all pertinent information in the medical record.

    Consistently document all aspects of patient care in a clear, concise and legible manner. Consistently document information in a chronological order at time of occurrence.
  • Provide direct patient care within scope of practice to ensure implementation of the Plan of Care. Consistently perform all nursing interventions on assigned patients, unless exceptions are discussed with the Physician. Consistently and willingly assist other team members in providing care to patients. Implement care considering growth and development factors appropriate to the age of the patient (Adults, Geriatrics). Inform and instruct patient/significant others on patient’s health/illness state and Discharge Instructions.

    Communicate all routines and health-related procedures performed to patient/significant others, as appropriate to the ages of the patient's growth and development through the life cycle factors (Adults, Geriatrics). Document patient/significant other’s response to patient teaching. Reinforce techniques/information provided by APN and providers needed for home care.
  • Be knowledgeable of and initiate emergency procedures. Initiate and carry out appropriate interventions in the event of a cardiopulmonary arrest, as appropriate to the ages of the patient's growth and development through the life cycle factors (Adults, Geriatrics). Verbalize knowledge of location and use of emergency equipment and disaster plan. Check emergency equipment and controlled substance records in accordance with established policies.

    Consistently report faulty/missing equipment upon discovery and replaces when indicated. Maintain supplies and equipment report all necessary information and/or unusual occurrences in accordance with established policies and procedures. Complete incident reports immediately in all required in situations. Consistently notify appropriate immediate supervisor of unusual and/or urgent occurrences when they develop. Report actions of others which negatively impact patient care when appropriate.
  • Administer medications in accordance with established Saint Peter’s Healthcare System guidelines. Administer correct medications/dosage/route to identified patient. Report medication errors as soon as discovered. Monitor and document adverse reactions to medications. Comply with controlled substance records in accordance with established policy. Demonstrate, knowledge/purpose of medication. Document medication administration appropriately in the medical record. Administer medications considering safety and effectiveness of medications relative to the patient’s age-related factors (Adults, Geriatrics).

    Maintain patient safety, comfort and cleanliness. Identify patient and medical record with accuracy before administering care as evidenced by absence of documented errors related to misidentification. Consistently evaluate patient’s capacity to safeguard self-prior to administering care and provide assistance as needed. Adhere to techniques outlined in Universal Precautions policy.
  • Demonstrate competency in performing all procedures within scope of practice and requirements for position, as appropriate to the ages of the patient's growth and development through the life cycle factors (Adults, Geriatrics), and an understanding of the range of treatment needed by these patients. Accurately take and record vital signs. Accurately obtain and document weight. Change dressing in accordance with established procedure unless instructed otherwise.
