Job DescriptionBased in the Monroe/Robbinsville clinic, the Social Worker will provide psychosocial screening, comprehensive assessment, counseling and case management to optimize outcomes for patients with cancer and blood disorders. The role also supports caregivers, facilitates support groups, and contributes to quality improvement initiatives.
Responsibilities- Comprehensive psychosocial assessment and treatment planning for patients and families.
- Coordination of care with physicians, nurses, advanced practitioners and multidisciplinary teams.
- Education and facilitation of monthly patient and family learning activities.
- Case management including community resource referral, financial and transportation assistance.
- Facilitation of 2–3 closed support groups per year via in‑person or virtual platforms.
- Documentation of assessments, care plans, collateral contacts and billing in the electronic medical record.
- Participation in departmental meetings, professional development and continuous quality improvement.
- New Jersey LCSW license required.
- Master’s in Social Work (MSW) from a CSWE accredited program.
- OSW‑C and SIFI certifications preferred.
- Spanish speaking preferred.
- One year or more of medical setting experience preferred; internship experience considered for entry.
- Experience with trauma, end‑of‑life and bereavement support is preferred.
- Computer proficiency for documentation and continuing education.
- Excellent clinical, communication, collaborative, organizational, analytical and interpersonal skills.
- Experience providing psychosocial services to patients with chronic illness.
- Proficiency in conducting in‑depth psychosocial assessment and delivering short‑term treatment.
- Interest and willingness to support clients coping with chronic and life‑threatening illness.
