About the Role

Optimal Care Hospice Registered Nurses (RNs) are skilled professionals who provide comprehensive nursing care, case management, and compassionate support to terminally ill patients and their families in home or facility settings. As a hospice RN, you will assess, plan, coordinate, implement, and evaluate individualized patient care plans.

Working closely with the hospice medical director, you will lead an interdisciplinary team—which may include hospice aides, medical social workers, chaplains, LPNs, music or massage therapists, and volunteers—to ensure patients receive holistic, high‑quality care.

What You'll Do
  • Conduct initial patient evaluations and ongoing reassessments to meet nursing care needs.
  • Manage your caseload, including assessment, planning, coordination, implementation, and evaluation of patient care plans.
  • Perform hospice-specific assessments, including:
    • Home safety evaluations
    • Appropriateness of the home environment for care delivery
    • Availability and capacity of support systems and community resources
  • Provide compassionate support to patients and families, including at the time of death. Conduct bereavement assessments as requested.
  • Maintain positive and professional communication with physicians, patients, families, facilities, visitors, and other team members.
Required Qualifications
  • Graduate of an accredited diploma, associate, or baccalaureate school of nursing
  • Current state license as a registered nurse
  • Minimum one (1) year experience as a nurse in a clinical care setting
  • Nursing skills as defined by accepted nursing standards
  • Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
  • Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles
  • Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines
  • Reliable transportation and valid automobile insurance coverage
  • Sit, stand, bend, lift, and move intermittently
  • Ability to lift at least 35 lbs and bear the weight of an average adult effectively
Desired Qualifications
  • BSN preferred
  • Experience as a nurse in a hospice or home health environment is preferred
  • Familiarity with Home Care Home Base (HCHB)

  • Service area:
    Monroe, Luna Pier, and surrounding communities
Schedule
  • Regular hours: 8:00 AM–5:00 PM, Monday through Friday
  • Weekend rotation: required on rotating basis
Pay Range

$80,000—$85,000 USD
