The LPN Clinic is responsible for providing patient care following established standards and practices. Assists physician, nurse, and staff with the daily coordination of patient care and patient access. Coordinates patient flow, schedules appointments, procedures, tests and referrals. May serve as a preceptor for clinical staff. Relies on guidelines to accomplish tasks. Relies on some experience and judgment to complete job.

Works under general supervision. May lead and direct the work of others within scope of practice.

Responsibilities
  • Direct Patient Care: Provide patient care utilizing the nursing process under the direction and supervision of the healthcare provider at all times.
  • Diagnostic Test Review: Review diagnostic test results and report abnormal findings or changes in patient condition immediately to the RN, physician, and appropriate healthcare team members.
  • Medication Administration: Administer prescribed medications, therapies, and treatments in accordance with practical nursing standards.
  • Safe Environment: Evaluate and maintain an environment that promotes patient and healthcare team safety at all times.
  • Confidentiality: Provide compassionate basic nursing care while maintaining patient privacy and confidentiality.
  • Plan of Care Implementation: Implement components of the plan of care within the LPN scope of practice and competency to promote desired outcomes.
  • Coordination of Care: Collaborate effectively with the supervising healthcare provider on the patient's plan of care and assist physicians, RNs, and health care team members with data collection, assessment, planning, intervention, and evaluation of patient and family care.
  • Advocacy: Act as an advocate for the patient, supporting psychological, ethical, spiritual, and cultural needs by collaborating with the RN and healthcare team.
  • Team Participation: Seek out healthcare team members as a resource to increase practice knowledge and actively participate in unit teams, work groups, or committees as requested.
  • Quality Improvement: Demonstrate knowledge of unit-based quality improvement initiatives and actively participate in improvement activities using standards, guidelines, and pathways for care delivery.
  • Other Duties: Perform any other duties as assigned.
Qualifications
  • Graduate of an accredited LPN program and high school diploma.
  • Proficient in English with strong verbal and written communication skills.
  • Computer skills and evidence of BLS certification.
  • Licensed as an LPN in the state of Louisiana or possess a work permit and BLS certification.
