Position: Medical Support Assistant (Advanced) - Patient  Aligned Care Team

Summary
The Medical Support Assistant (Advanced) is assigned to a Patient Aligned Care Team (PACT) model for Mental Health or a Specialty Care Setting involved in a PACT model. The PACT team typically consists of a primary care provider (MD, NP, PA), a registered nurse care manager, a clinical associate (LPN or medical assistant/health technician), and an administrative associate (MA/MSA/health technician).

Responsibilities
  • Coordinate with the patient care team to review clinic utilization using reports such as the Clinic Utilization Statistical Summary, ensuring the clinic setup effectively supports patient needs and making necessary adjustments.
  • Develop and maintain efficient communication with the patient, the interdisciplinary team, VA medical centers, and other agencies, including assisting with outpatient discharge communications.
  • Facilitate and process secure patient messaging.
  • Notify patients of normal lab results and develop a tracking system for follow‑up care (consults, tests, etc.).
  • Participate in team huddles, meetings, and operational problem‑solving to manage and plan patient care.
  • Set priorities and deadlines, adjusting workflow to meet team and patient needs.
  • Support the team by reinforcing the plan of care, self‑help solutions, and entering appropriate information into the electronic record.
  • Monitor pre‑appointment requirements to assure readiness for patient visits or procedures (e.g., X‑ray, lab work).
  • Manage the electronic wait list, verifying accuracy and resolving issues.
  • Evaluate patient information and clinic schedule lists to determine patient eligibility.
  • Educate providers about shared patients and bring this information to the provider’s attention.
Work Schedule and Benefits

Day Shift: 7:30 am – 4:00 pm, Monday – Friday (subject to change based on facility needs). Salary is competitive with regular increases. Standard benefits include 37–50 days of annual paid time off, 12‑week paid parental leave after 12 months of employment, and access to VA retirement, health, vision, and dental plans.
