Overview
The Reg Nurse CC is responsible for providing professional nursing for patients following established standards and practices. Assists physicians and staff with the daily coordination of patient care and patient access. Coordinates patient flow, schedules appointments, procedures, tests and referrals. May serve as preceptor for clinical staff. Relies on experience and judgment to accomplish responsibilities. Works under minimal supervision. Creativity is expected to perform job.
May lead and direct the work of others within scope of practice.
- Patient Care: as described, including thorough assessments, planning of care, patient education and discharge planning, and tailoring care to individual needs.
- Perform psychosocial/physiologic assessments that integrate changing data; document and respond to patient needs with independent clinical judgment.
- Develop and adapt plans of care based on trends, including education and discharge planning; incorporate cultural/ethical/spiritual considerations.
- Participate in planning routine transitional health care needs (treatment options, patient placement options, end-of-life options) as appropriate.
- Respond promptly to changes in patient needs and provide holistic care using knowledge from education and experience.
- Educate patients and families by tailoring teaching strategies to individual needs; collaborate with patients/families to identify realistic outcomes.
- Ensure efficiency in care delivery; identify priorities and provide solutions as needed.
- Advocate for patient rights; address conflicts and seek resolution when needed.
- Identify variances from expected outcomes and revise the plan of care accordingly.
- Document all patient care activities accurately; assist the health care team with documentation as needed.
- Coordinate care with health care team members, patients and families to maximize outcomes.
- Delegate appropriate tasks and participate in staff assignments to achieve productivity; integrate unit initiatives into practice.
- Actively participate on unit teams, work groups and committees; anticipate needs and assist others when possible.
- Serve as a clinical resource, mentor and preceptor; provide leadership and guidance as needed.
- Participate in quality improvement initiatives and apply guidelines and pathways for care delivery; incorporate evidence-based practice into nursing care.
- Participate in other duties as assigned and serve on committees as needed; act as a backup for others in the department.
- Graduate of an accredited school of nursing
- Per license requirements
- Proficient in the English language, verbal and written communication skills, computer skills
- Current Louisiana State license as RN; BLS
