- Annual TB Questionnaire required within 6 months of start.
- Health statement signed within 1 year of start stating ability to work without restrictions.
- Proof of negative drug screen within 30 days prior to start.
- Various background checks and verifications required including criminal background, education, employment, and driving record.
- Completion of FMOLHS Skills Checklist, Competency Exam, Core Competencies, and other required education packets and forms.
