Expected Pay
Expected pay for this position is $29.30 - $34.47/hour. Actual pay will be determined by experience, skills, and internal equity. This is a salaried position.
The Ambulatory Complex Care Manager promotes optimal clinical and resource outcomes by assessing, planning, implementing, coordinating, and monitoring interventions, and by developing a comprehensive plan of care for patients. The manager performs ongoing communication and collaborates with the interdisciplinary care team to promote quality of care. Health care services are coordinated on behalf of the patient to optimize outcomes and satisfaction.
The manager addresses complex psychosocial and behavioral health issues, population management needs, and transition plans. The manager works with the Care Management Division leadership team to ensure alignment with OSF’s strategic goals.

Bachelor’s degree in Social Work (BSW) - Licensure/Certification
:
Social Work licensure in Illinois and/or Michigan - Compliance
:
Must comply with state licensing and certification requirements for the state of practice. All BSW mission partners must obtain licensure within 4 years from date of hire. BSWs must identify a clinical supervisor by day 91 of hire and complete 3‑year supervision. If supervised hours are already documented, they will be deducted from the 4‑year requirement. MSW candidates must begin licensing application by day 91 and obtain licensure within 1 year from hire. - Experience
: 3 years of experience caring for patients - Skills & Knowledge
:
Broad knowledge of chronic illness and medical/community resources; working knowledge of a clinical EMR system and Microsoft Office; flexible schedule—including weekend, holiday, and on‑call rotations; independent judgment and decision making with little supervision; substantial interpersonal skills to maintain relationships across departments; excellent time management; positive, empathetic communication with diverse patients and families. - Other Requirements

Ability to work independently and make decisions with minimal supervision.
- Master’s degree in Social Work (MSW)
- Experience with chronic, complex illnesses in adult and/or pediatric care management
