Care Rx is looking for a Clinical Consultant Pharmacist to join our Moncton team. The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long term care, supportive living, and other congregate living communities. The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, site policies, internal Care Rx policies, and the Ministerial Order for Clinical Pharmacy Services.

Role Accountabilities:
- Act as liaison between long term care homes and retirement communities and Care Rx Pharmacy to ensure optimal Resident care, Pharmacy Services, and Clinical support
- Travel to LTC homes and retirement communities
- Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams to develop/maintain strong partnerships
- Complete medication reviews in currently serviced retirement communities and long-term care homes serviced by Care Rx Pharmacy
- Complete all required documentation and billing processes as required by provincial regulations, Provincial College of Pharmacists, and other federal and provincial standards and regulations
- Provide education to Residents and staff on various health-related topics including the Meds Check program and attend community-based wellness fairs and other events as required
- Conduct education sessions within Long Term Care Homes and Retirement Communities within scope of practice, as applicable (e.g. policy and procedure, web portal, eMAR, influenza protocol, etc.)
- Support Care Rx Pharmacy policies and procedures within retirement communities and long-term care homes
- Complete medication audits on-site in LTC Homes and Retirement Communities as required
- Ensure regular on-site visits are made to all assigned clients
- Attend/participate in annual professional conferences and trade shows, when applicable
- Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable
- Be an education, policy and pharmacy initiative resource
- Be available for special clinical team projects as required
