- Conduct comprehensive home visits, perform assessments, develop and adjust individualized care plans, administer medications and therapies per physician orders, and monitor vital signs and symptom trajectories.
- Coordinate with physicians, pharmacists, physical and occupational therapists, and social workers to ensure seamless, patient‑centered care.
- Educate patients and families on disease management, medication safety, fall prevention, nutrition, and activity planning.
- Document care accurately, maintain compliant records, and leverage telehealth and remote monitoring when appropriate.
- Prioritize safety, infection control, and adherence to regulatory standards.
Guaranteed 40 hours per week, continuous skill development, and opportunities for career progression within a traveling home health program.
Compensation & BenefitsCompetitive weekly pay range of $2,062 to $2,113, sign‑on bonus, robust housing assistance, and contract extension opportunities. 24/7 support is provided during travel assignments, with remote monitoring and real‑time guidance.
