We are seeking a highly skilled and compassionate MDS Coordinator RN or LPN to join our team at Allure of the Quad Cities, a premier skilled nursing and healthcare center in Moline, Illinois. As an MDS Coordinator, you will play a critical role in ensuring the highest quality of care for our residents by coordinating the care planning process and facilitating communication among healthcare professionals.
Responsibilities- Coordinate care planning:
Collaborate with healthcare professionals to develop and implement individualized care plans that meet the unique needs of each resident. - Facilitate communication:
Ensure effective communication among healthcare professionals, residents, and families to ensure seamless transitions and optimal care. - Gather and analyze data:
Collect and analyze resident data to identify trends and areas for improvement, and provide insights to inform care decisions. - Conduct MDS assessments:
Conduct comprehensive assessments to identify resident needs and develop plans to address them. - Provide education and training:
Educate and train other healthcare professionals on care planning, MDS policies, and procedures. - Maintain accurate records:
Document resident data, care plans, and progress accurately and efficiently. - Participate in quality improvement initiatives:
Contribute to quality improvement initiatives aimed at enhancing resident outcomes and improving our care services.
- Education:
RN or LPN degree from an accredited institution (or equivalent). - Experience:
1 year long‑term care experience preferred. - Licensure:
Current RN or LPN licensure in the State of Illinois. - Certifications:
BLS certification is required.
