The Licensed Nurse is responsible for supervising the personal care staff; coordinating all personal care services provided to residents by personal care staff; promoting the health and well‑being of each resident; and reporting observed changes in resident condition or health to the Executive Director and the resident's responsible party.

- The Registered Nurse performs a thorough health assessment on all new residents upon admission to the community. The RN will review admission orders and resident profiles to ensure the accuracy of information (including but not limited to diet, DNR status, allergies, diagnosis, services, etc.)
- The Registered Nurse is responsible for monitoring the health status of all current residents through assessments and care conferences scheduled quaterly or more frequently as required (30‑day reviews, 90‑day reviews, changes in condition). Resident assessments will be completed by the RN (consulting with ED, HSD, and RCC).
- This RN will ensure care plans will be entered into Eldermark and printed by due dates and as needed for changes in condition, and the RN will monitor compliance with those care plans by all staff.
- This monitoring includes actions required to improve their health status and reading of all alert charting from daily reports.
- This monitoring is to include, but not be limited to: medication side effects, activities, meals, complaints, cognition and behaviors, ADLs, and special requests.
- Using this information, the RN will coordinate and attend resident care conferences and service plan reviews, as requested by the Executive Director, HSD, or RCC, with a focus on assuring that the service plans are developed to meet the resident's needs.
- The Registered Nurse will review all doctor orders (RN check) for psychotropic medications. The RN will conduct a focused assessment and enter a nursing note per state regulations regarding psychotropic medications.
- The Registered Nurse will react immediately to suspected resident abuse, including a thorough investigation and notification of appropriate parties as needed.
- The Registered Nurse will perform skin assessments and enter weekly skin notes for residents with skin issues. The RN will ensure the skin protocol is followed, including checking for or entering skin treatments on the MAR.
- The Registered Nurse will communicate with PCPS, Home Health agencies (PT, Wound care, etc.), and hospice in accordance with residents' needs.
- The Registered Nurse will review monthly monitors for all residents, coordinate with PCPs, and enter chart notes accordingly (vitals, weights, pain, skin, falls, hospitalizations, coordination of care, diabetes, blood thinners, supportive devices with restraining qualities, etc.)
- The Registered Nurse will ensure delegations are in place for residents needing subcutaneous injections (such as insulin‑dependent residents). The Registered Nurse will ensure delegations are done in accordance with Oregon Regulations‑ Board of Nursing (chapter 851, Division 47).
