Position Summary

Reporting to the Stroke Coordinator, the Nurse Navigator provides expert nursing consultation and practice oversight, functioning as a liaison between medical staff, nursing staff, family members and support services. The role coordinates multidisciplinary care based on evidence‑based practice, provides stroke education, participates in stroke‑related research, and oversees patient care including post‑discharge follow‑up. The Navigator collaborates with Stroke Program stakeholders to meet certification requirements, review data and lead performance improvement projects, and incorporate patient experience into all care aspects.

Qualifications
  • Education & Experience
    • Associate’s Degree in Nursing required;
      Bachelor’s Degree preferred
    • Minimum two years of neuroscience experience required
    • Analytical and communication skills necessary, both verbal and written
    • Fluent in spoken and written English
    • California RN license required; BLS and ACLS certifications required
    • Valid NIH Stroke Scale (NIHSS) certification or completion within three (3) months of hire
    • Advanced Stroke Life Support (ASLAS) certification or completion within three (3) months of hire
    • Preferred certifications:
      Stroke Certified Registered Nurse (SCRN), Certified Neuroscience Registered Nurse (CNRN), or Neurovascular Registered Nurse (NVRN)
  • Physical Demands
    • Prolonged standing/walking, lifting up to 50 lbs, frequent reaching, stooping, bending, kneeling, crouching
    • Manual dexterity and mobility required
    • Ability to move quickly and consistently perform duties under variable conditions
  • Vision Requirements
    • Ability to adjust vision sufficiently to perform all position accountabilities
  • Working Conditions
    • Regular exposure to risk of contagious and blood‑borne diseases, unpredictable situations, and emergency or crisis events
    • Exposure to radiation hazards, radioactive substances, and biohazards in outpatient, oncology, operating room (limited) and inpatient environments
    • Frequent exposure to hazardous chemicals, bodily substances, extreme noise, radiation, laser, and related equipment handling
    • Irregular hours and increased stress due to multiple calls and inquiries
