Stroke Program Nurse Navigator (Full Time, Days)

Doctors Medical Center Modesto – located in Modesto, CA. We are a full‑service comprehensive health‑care facility dedicated to delivering high‑quality medical services.

Position Summary: Report to the Stroke Coordinator and serve as the liaison among medical staff, nursing staff, families, and support services. Coordinate multidisciplinary care using evidence‑based practice, provide stroke education, participate in research, and oversee patient care including post‑discharge follow‑up. Work collaboratively to meet program certification requirements and drive performance improvement.

Key Responsibilities:

• Consult on nursing practices and oversee quality of care for stroke patients.
• Coordinate multidisciplinary care teams and ensure evidence‑based protocols are followed.
• Provide stroke education to patients, families, and staff.
• Participate in stroke‑related research and quality improvement projects.
• Manage post‑discharge follow‑up and data collection for program performance.

Minimum Qualifications:

• Associate’s Degree in Nursing (Bachelor’s preferred).
• Minimum two years of neuroscience experience.
• Active California RN license.
• BLS, ACLS, and current NIH Stroke Scale (NIHSS) Certification (or completion within 3 months of hire). Advanced Stroke Life Support (ASLS) Certification required (or completion within 3 months of hire).
• Strong analytical, communication, and English language skills.

Preferred

Certifications:

Stroke Certified Registered Nurse (SCRN), Certified Neuroscience Registered Nurse (CNRN), or Neurovascular Registered Nurse (NVRN).

Physical and

Working Conditions:

• Prolonged standing/walking, lifting up to 50 lbs, frequent reaching and bending.
• Exposure to contagious and blood‑borne diseases, hazardous chemicals, radiation, and loud noise.
• Irregular hours and occasional emergency situations.
