- Provides orientation to new patients to the programs’ rules, policies, procedures and patients’ rights.
- Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans within time guidelines and regulatory requirements.
- Involve patients in the assessment, treatment planning and intervention aspects of treatment.
- Administer the biopsychosocial assessment at intake and on an ongoing basis.
- Recognize and utilize patients’ Strengths, Needs, Ability, and Preferences (SNAP) to develop individualized Treatment Plans.
- Determine the clinical necessity of counseling services based on assessment.
- Monitor and document any change of circumstances, especially relapse indicators.
- Comply with Medical Q.A and Clinical Risk Management Policies.
- Evaluate patients for high‑risk conditions and ensure appropriate care.
- Administer screening tools (BAM‑R, C‑SSRS, GAD‑7, PHQ‑9) to assess mental/emotional status.
- Evaluate risk factors and determine treatment appropriateness.
- Recommend immediate referral to specialists when necessary and obtain waivers for information release.
- Notify Medical Directors, Physician Extenders and Regional Clinical Leadership of high‑risk patients.
- Refer high‑risk patients per policy and schedule case conferences.
- Review physician notes and ensure high‑risk factors are documented.
- Advise staff of high‑risk patients and circulate minutes.
- Provide professional counseling and referral services, including specialized services and community resources.
- Deliver individual and group counseling using evidence‑based modalities such as CBT and MI.
- Schedule regular counseling sessions and keep accurate records.
- Document counseling entries required by regulations and policies.
- Engage in community relations and outreach efforts.
- Attend required training, including Physician Training of Counselors, RCL training, and case conferences.
- Collect research data when requested.
- Observe urine sample collection when required.
- Attend team meetings and complete all training courses timely.
- Other duties as assigned.
- Associate degree in a relevant field and required registration/credentials for counseling.
- Associate degree or majority of approved counselor certification courses, license or certification.
- Certification by a recognized certifying organization; proof of registration required immediately upon employment.
- 2‑5 years of experience or master’s level education.
- Licensed, license‑eligible or certified; clinical skills effective in complex cases.
- Certification as an Alcohol and Other Drug Counselor with a CA approved agency or current credentials as licensed.
- Proof of licensure or certification required immediately upon employment.
