Days Position Overview Virtus Healthcare Staffing is seeking a dedicated Physical Therapist to join a partner's interdisciplinary team in Modesto, CA. This role is ideal for a clinician who is passionate about providing patient-centered, rehabilitative, and restorative care to a frail or elderly population.

The successful candidate will be a key member of the interdisciplinary team, directly involved with care planning and collaborating with the entire clinical team. This is a pivotal role that requires a focus on maximizing participant independence and safety through expert physical therapy services delivered both in the facility and at home.

Qualifications Required:
• Bachelor's Degree in Physical Therapy plus five (5) years of Physical Therapy experience.
• Valid California Physical Therapy License, or proof of License eligibility.

Preferred:
• Master's Degree in Physical Therapy or Doctorate in Physical Therapy.
• Two (2) years of physical therapy experience in a clinical setting with a frail or elderly population.
• Current CPR certification.
• Degree earned outside of the U.S. evaluated to the US equivalent.

Key Responsibilities Patient Care & Clinical Excellence
• Conduct physical therapy assessments in the facility and in participants' homes to define care needs, preferences, and goals.
• Deliver and document physical therapy interventions in accordance with physician orders and care plans.
• Provide active and passive exercise, muscle re-education, gait, and functional training for activities of daily living (ADLs).
• Provide training and instruction to staff, participants, and caregivers/families in the use and care of therapeutic equipment.

Collaboration & Leadership
• Actively participate in interdisciplinary team (IDT) meetings to develop participant care plans.
• Work alongside the Primary Care Physician, rehabilitation team, and other IDT members to maximize participant independence and restore function.
• Supervise Therapy Assistants, including providing the training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP).
