• Conduct needs assessments to determine eligibility for the program based on guidelines and requirements
• Prepare and maintain case records for assigned clients, entering pertinent information into the Homeless Management Information System (HMIS)
• Establish comprehensive goals for eligible clients and provide transportation to meetings related to the case plan
• Interview, accept, and provide comprehensive case management services for an assigned caseload
• Conduct mentoring and counseling sessions with clients and/or staff to evaluate progress

• Bachelor’s degree or greater from an accredited college or university in Social Work, Behavioral Science, or a related field
• Minimum of three years progressively responsible experience providing direct case management social services
• Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities
• Valid Driver’s License
• Ability to work in a normal office environment with minimal physical discomforts

• Experience in structured case management services
• Familiarity with Homeless Management Information Systems (HMIS)
• Strong mentoring and counseling skills
• Knowledge of community resources and referral processes
• Proven ability to develop and implement program plans/goals
