The Licensed Practical Nurse (LPN) performs assessments and selected medical nursing procedures in clients' homes, under the guidance of the RN Supervisor. The LPN is responsible for implementing nursing care based on the plan developed by a Registered Nurse (RN) in accordance with physician's orders.

- Perform selected nursing procedures within the scope of LPN/LVN practice (e.g., tracheostomy care, colostomy care, enemas, douches, catheterizations, dressing changes, phlebotomies) in the home setting, commensurate with experience and education.
- Provide patient and family teaching according to the plan outlined by the RN.
- Monitor patient vital signs and record measurements and observations.
- Monitor and record intake and output; report significant changes immediately.
- Observe, assess, and document patient physical and mental condition; report any changes.
- Administer medications as instructed by the physician and as ordered by the RN; document in the patient’s file.
- Collaborate with the patient, family, and healthcare team to coordinate care.
- Maintain confidentiality due to access to sensitive information.
- Maintain regular, predictable attendance and flexibility to meet agency needs.
- Follow all Medicare, Medicaid, and HIPAA regulations and requirements.
- Adhere to all regulations, policies, procedures, and standards.
- Performs other duties as assigned.
- Graduate from an accredited school of practical nursing
- Licensed as a vocational/practical nurse in the state of practice
- At least two (2) years of clinical practical nursing experience
- Ability to make home visits and provide nursing care in the home environment
- Ability to complete necessary documentation accurately, legibly, and timely
- Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
- Must have reliable transportation, current driver’s license and appropriate automobile insurance; or ability to use public transportation
