Provides individualized assistance to patients, families, and caregivers to achieve patient-centered, evidence-based care; acts as a link between patients and care providers to facilitate clinical services and empowers patients to be an active part of the care team; works with patients to overcome obstacles to disease management by utilizing all available resources, both internal and external, to improve care outcomes; serves as Patient Navigation to USA Health Physician Group specialty areas/service lines by providing information, support, encouragement and assistance to patients and family caregivers to overcome barriers in receiving care;
serves as a guide to patients and family caregivers throughout their care journey; maintains frequent contact with patients and family caregivers in person, by telephone, in writing, and by email; maintains communication with the patient’s care team as well as their primary care provider to facilitate timely identification of patient issues to prevent care delays, unnecessary emergency room visits and hospitalizations;
stays abreast of all aspects of service line care and treatment methods; provides patient and family caregivers with information regarding access to resources and community support; stays abreast of available resources from the pharmaceutical industry, charitable, community and faith-based organizations; maintains navigation records in a timely manner using a database; maintains confidentiality and exhibits professional conduct when interacting with patients, family caregivers and medical professionals;
organize patient schedules and manage appointments to facilitate timely services; facilitate scheduling of appointments for clinical consultations and support services inclusive of the medical care team and support services; follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources; match unmet needs with referrals to internal and external resources, including but not limited to social worker, dietitian, financial counselor, community partner agencies;
provide emotional and educational support for patients and families; identify alternative payment resources for the non-insured and underinsured and assist with enrollment; serve as a patient advocate on the multidisciplinary team; transition patients from one phase of care to the next, including self-management and independence in self-care; articulate service and customer relations improvements to ensure patient retention; participate in service line performance improvement activities;
maintain regular and prompt attendance; work schedule as defined and overtime as required; related duties as required.
Employees must be in a regular position, working 20 hours or more per week (.50 FTE or greater) to qualify for benefits.
QualificationsHigh school diploma or equivalent and two years of related experience; or successful completion of a vocational program in a healthcare related field and one year of related experience. Higher education may substitute on a year-for-year basis for the required experience.
