Case Manager - Outreach - SSVF (JR 5322)
Path (People Assisting The Homeless) is seeking a Case Manager to join the SSVF Team at the RHC Office in Los Angeles, CA. The role is full-time, non-exempt, and requires reliable transportation.
Compensation
: $23.24 to $28.28 per hour, 24 pay periods per year.
- Conduct intake and individualized needs assessment for all participants and develop an Individualized Service Plan (ISP) that addresses housing goals and other personal goals.
- Provide referrals, linkages, and support to resources that help participants achieve ISP goals.
- Complete program-specific assessments (e.g., VI-SPDAT).
- Evaluate strengths and challenges and conduct a 90‑day review with participants after initial intake.
- Maintain documentation standards and complete case file notes in a timely manner.
- Maintain confidentiality of participant files per HIPAA and applicable guidelines.

Maintain ethical and professional standards.
- Two (2) years of experience in healthcare, case management, homeless services, or a related field working with vulnerable populations.
- All levels of education and experience are welcome.
- Employment eligibility verification.
- CPR/First Aid training or willingness to obtain.
- Successfully complete Tuberculosis Test, Background Screening, and Drug Test as conditions of hire.
- Reliable transportation, valid driver’s license, proof of insurance, and ability to qualify for PATH’s insurance coverage.
