The LVN Field Clinician is a Licensed Vocational Nurse (LVN) who works directly in the patient’s residential or other skilled care continuum of health setting to provide skilled nursing wound care to the adult patient as directed by a Woundtech Provider [i.e., Woundtech Physician, Doctor of Podiatric Medicine (DPM), Nurse Practitioner (NP), Physician Assistant (PA)] or Registered Nurse clinician.
Responsibilities- Assess patient/caregivers’ needs in relation to patient’s presenting wound condition and prioritize care of the wound based on these needs.
- Ensure quality and safe delivery of home health/residential or facility wound care services to assigned patients.
- Implement plan of wound care formulated by physician, provider, or Registered Nurse.
- Perform direct wound care of assigned patients within the limitations of their licensure scope and per organizational policies.
- Identify patient/family or caregiver learning needs and implement appropriate measures to meet them.
- Provide accurate and timely documentation consistent with plan of care.
- Confirm patient/family or caregiver understanding of care and treatments received.
- Use equipment and supplies effectively and efficiently.
- Recognize emergency situations and take appropriate actions.
- Provide patient updates to Woundtech Provider, CM, patient’s Primary Care Provider (PCP) and other key stakeholders as needed.
- Demonstrate organizational skills and flexibility to manage multiple tasks.
- Independently troubleshoot minor telehealth technical difficulties and elevate to technical staff when appropriate.
- Communicate in a clear and understandable manner using appropriate verbal and non-verbal styles.
- Actively participate in departmental activities to improve training methods and achieve company goals.
- Maintain current clinical, technical, and professional competencies.
- Attend required meetings and participate as a committee member as requested.
- Assist with other projects and assignments within organization as needed.
- Other duties as assigned.
- Graduate of an accredited practical/vocational nursing educational program approved by an individual state board of nursing-governing body.
- 2-3 years’ experience at the nursing practice level.
- Current, active, unrestricted license as a Licensed Practical Nurse (LPN) or a Licensed Vocational Nurse (LVN) in respective state.
- Prior wound care experience preferred.
- Wound care certification preferred.
- Telemedicine experience preferred, but not required.
- Excellent verbal and written communication skills in a professional environment.
- Ability to lift 50 lbs. or more.
- Must have full range of mobility.
- Must be able to reposition patients.
- Valid and unrestricted driver’s license and willing and able to see patients within a 50-mile radius of their home.

- Ability to set priorities while managing multiple tasks.
- Detail oriented with excellent organization skills.
Vehicle/Home, health care residential, and skilled nursing settings.
