Job Summary Oak Glen Post Acute is seeking a compassionate and dependable
** Licensed Vocational Nurse (LVN)
** to join our skilled nursing team. The LVN will provide high-quality nursing care to residents while working collaboratively with RNs, physicians, and interdisciplinary staff to support resident health, safety, and comfort.### Responsibilities
* Provide direct nursing care to residents in accordance with facility policies and procedures
* Administer medications and treatments as ordered by physicians
* Monitor residents’ conditions and report changes to the RN or physician
* Accurately document care, treatments, and resident responses in medical records
* Assist with admissions, discharges, and care planning
* Ensure compliance with infection control, safety, and regulatory standards
* Support residents and families with professionalism, empathy, and respect
* Collaborate with the interdisciplinary care team to deliver person-centered care### Qualifications
* Current
** Licensed Vocational Nurse (LVN)
** license in the state of California
* CPR certification (BLS)
* Experience in skilled nursing or long-term care preferred
* Strong clinical, communication, and documentation skills
* Ability to work effectively in a fast-paced healthcare environment
