** Job Summary
** The LVN II is accountable and responsible for selected aspects of patient care for a designated group of patients within the LVN scope of practice, in collaboration with multidisciplinary teams. The LVN practice is assigned, delegated, and supervised by a RN or MD who has overall responsibility for the coordination and management of care.
This position is day shift (8am-4:30pm), three week days per week (24 hours per week).
- The LVN II will provide administrative support to Case Managers with orders, including but not limited to: DME and supplies, procedure authorizations and/or appointments, to ensure complete patient care.  - Follow up with vendors to ensure orders have been received and obtain updates on authorizations and delivery status   - Provide Polysomnography results as requested by Providers  - Assist and support our providers and patients in all Sleep clinics, including scheduling appts, Hosting telehealth visits, coordinating care and discharging patient  - Propose orders for physicians to advocate for patients and families for needed services, resources, or medical orders  - Assist Physicians in scheduling peer to peer appointments to seek approval on denied or redirected authorizations for Polysomnography  - Firsthand communication with the authorizations department to ensure prior authorizations have been obtained, and follow up within a timely manner to ensure effective turnaround time for patients

** Minimum $30.94 Midpoint $40.99 Maximum $51.05

Required:

Minimum one (1) year of LVN experience in an acute care or ambulatory setting.

Graduate of an accredited school for Licensed Vocational Nursing.

Basic Life Support (BLS);
Licensed Vocational Nurse - CA (LVN);
Ped Emerg Assess Recog & Stabil (PEARS) - must be completed within 6 months of hire.
