Pelican Bay State Prison, Crescent City, CA. The position involves providing comprehensive nursing care in a correctional facility environment, supporting medication assisted treatment, hepatitis C treatment, and other health care initiatives under the California Correctional Health Care Services.

Responsibilities- Organize and provide necessary nursing care for patients participating in the Complete Care Model (CCM) initiatives such as Medication Assisted Treatment (MAT) and Hepatitis C Treatment (HCV).
- Serve as Care Manager for patients with chronic care management needs and coordinate interdisciplinary communication.
- Assess patients for side effects, signs/symptoms of recent drug or alcohol use or withdrawal, mental health, and/or medical conditions that interfere with treatment.
- Facilitate recovery, physical health and wellness, medication management groups, and promote therapeutic relationships.
- Act as a mentor for patient peer development and actively participate in Peer Education Programs.
- Attend primary care meetings and population management sessions, collaborate with the Care Nurse on care visits and care planning.
- Administer injections, provide high-level education to patients, and serve as a tele‑presenter for the Telemedicine Addiction Provider.
- Address ongoing health care needs of patients and promote timely, efficient, and appropriate use of health care resources.
- Possession of a current license as Registered Nurse in California. Applicants who do not meet this requirement will be admitted to the examination, but must secure the required license before they will be considered eligible for appointment.
- To obtain eligibility for the Registered Nurse, Correctional Facility before applying for the position, you must first take and pass the examination offered through Cal Careers.
