In-Home Health Clinician - Physician, NP, PA; Part Time Fresno​/Inyo County, CA

Job Overview

Signify Health, part of CVS Health, is seeking a Clinician (Nurse Practitioner, Physician Assistant or Physician) to provide In‑Home Health Evaluations for patients in Fresno, CA. The role includes 45‑minute visits, medication & medical history review, physical evaluation, and optional diagnostic tests. Clinicians use an iPad and an intuitive clinical workflow. You will be part of a national network of purpose‑driven clinicians dedicated to improving health outcomes.

You’ll play a critical role in building trusted relationships to make people healthier, helping bring flexible, comprehensive and personalized health evaluations right to people’s front doors.

The visit lasts about 45 minutes and includes time to connect one‑on‑one, answer health questions, review medications and medical history, perform a physical exam, and conduct diagnostic tests if ordered. You’ll not prescribe medicine, order lab tests or alter treatment regimens.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required for NP/PA).
• Multi‑state licenses (preferred).
• Able to cover the following primary locations:
