Delmar Gardens Private Services is seeking a Full-Time Registered Nurse (RN) Case Manager for our beautiful new St. Ignatius Hall located in north St. Louis County. Please contact us at your earliest convenience to learn more about this unique opportunity.

Job Responsibilities- Assist the men with scheduling and coordination of health care specialists
- Accompany Jesuits to physician or specialist appointments as needed
- Report changes in condition and relevant health updates to the nurse on duty, Care Coordinator Nurse, and Jesuit leadership
- Manage admissions and readmission, including communication of condition, treatments, and medications
- Ensure care plans are accurately and promptly communicated to the Care Coordinator Nurse and/or floor nurse on duty
- Assume full responsibility for assigned cases
- Complete documentation, charting, and planning in accordance with prescribed procedures

Requirements
- Current Registered Nurse (RN) license in the State of Missouri
- CPR certification required
- Minimum of one (1) year experience in an adult medical/surgical unit
- Six (6) months of home care experience preferred
