Hourly Pay (Not Per Visit) Guaranteed 40 hours/week

Bonus & Equity Eligibility

Conduct skilled nursing assessments in the home and contribute to interdisciplinary care plans

Deliver and document care aligned with physician orders and patient needs

Administer medications, perform ordered treatments, and ensure a safe home environment

Educate participants and their families on disease management, self-care, and preventative health

Coordinate care transitions between hospitals, skilled nursing facilities, and home

Track and manage visit schedules in coordination with the Home Health Manager

Graduate of an accredited School of Nursing

Active, unrestricted RN license in California

1+ year of nursing experience with elderly or frail populations (home health experience preferred)

Reliable transportation for local travel

No security clearance required

Visa sponsorship not available

Travel: Local home visits; occasional travel required
