We are looking for a Licensed Practical Nurse (LPN) to render or assign resident care according to resident needs under the direction of the supervisor or director of nursing. Maintain a home-like, person-centered care environment for all residents. As an interdisciplinary team member, meet the physical, psychological, and social needs of residents within Americare communities.

• Serve as a liaison between Home Health and Americare communities to provide appropriate care.
• Provide nursing services within your scope of practice to implement nursing objectives, standards of care, policies and procedures; review and revise as necessary.
• Ensure nursing care standards follow written guidelines; collaborate with the Director of Nursing and Home Health Administrator.
• Give clinical guidance to other nursing staff (PCA, CNA, CMT) as necessary.
• Make rounds to assess residents and staffing needs; visit residents daily.
• Ensure nursing personnel are complete under the direction of the Director of Nursing.
• Document nursing assessments, notify physicians/families of changes in condition, obtain orders, and implement orders.
• Maintain necessary written documents (Nursing Notes) and communicate with facility Administrator and Director of Nursing.
• Be available for call as needed for nursing care.

• Graduate of an approved school of nursing.
• Current LPN license and state registration with ongoing education.
• Basic knowledge of Home Health care.
• Detail-oriented with the ability to maintain accuracy and confidentiality.
