Per Diem/Weekends Hospice Licensed Vocational Nurse (LVN/LPN)Hourly Rate $35.00 - $38.00 plus mileage reimbursement

POSITION SUMMARYThe Per Diem/Weekend LVN position offersan opportunity for the LVN to work u
nder the direction of a Registered
Nurse and
provide skilled nursing and palliative care to patients at their place of residence. The LVNworks cooperatively with other Interdisciplinary Team members and providesintermittent visits or shifts of skilled nursing care, including pain management/symptom control, and nursing observations. The LVN provides information about the expected course of illness, care techniques and other health measures to patients, family members or primarycaregivers. The LVN participates in the care plan development and implementation.
Must be able to utilize computers and be comfortable with electronic medical records.
- Graduate of an accredited schoolof vocational nursing.
- Current LVN license within the State of California. Certification in Hospice and Palliative Nursing encouraged.
- At least one year of LVNexperience.
- Valid driver's license, automobile insurance, reliable and well maintained transportation.
- Spanish speaking a plus, but not required
