As a Nurse Practitioner you will:- Partner with local communities and senior living organizations to build our presence
- Help shape market strategy and patient panel development
- Influence clinical workflows and care practices as the market grows
- Build trusted relationships within care ecosystems across St. Louis
- Serve as a key ambassador for Bloom in one of our newest territories
- Deliver comprehensive primary care through at-home and senior living visits
- Manage an empaneled patient population and provide approximately 10 visits/day
- Collaborate with an interdisciplinary team including specialists, pharmacists, social workers, and care managers
- Use population health insights to support better outcomes
- Participate in ongoing training and development (including occasional travel to Denver)
- Build strong partnerships with senior living leaders, hospital systems, and community organizations
- Flexible schedules: 3-, 4-, or 5‑day workweek options
