Siena Terraces, a Generations Healthcare facility, is looking for a Wellness Coordinator, LVN or RN, to join our team.
Position OverviewUnder minimal supervision, responsible for coordinating resident care and medication management under the medical direction and supervision of the residents' attending physicians in order to ensure the resident remains as independent as possible for as long as possible. Will assist the resident and his or her family members in maintaining the physical and emotional health of the resident and will liaison with hospital personnel, physicians, community organizations and other health related service agencies to provide care to the residents.

Essential Functions- Communicate with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies on the care needs of all residents at the community.
- Coordinate incidental medical, dental, vision, hearing, and podiatry care for residents.
- Perform nursing duties as directed and in accordance with the appropriate scope of the Nurse Practice Act, including administering medications and performing treatments as ordered by physician.
- Oversee and provide necessary training for Medication Technicians to ensure competent delivery of medications to residents and understanding of emergency procedures, documentation standards, and acceptable treatments.
- Administer resident pre-admission assessment prior to each resident's move in date and complete routine and as-needed reassessments in accordance with company policy and state regulations. Maintain accurate and up to date resident care information.
- Provide care staff with copies of the most current resident service plans and staff tasks sheets as needed.
- Communicate all relevant resident information to physicians and other healthcare professionals.
- Complete thorough documentation in resident chart on an ongoing basis according to company policy and state regulations.
- Monitor the community's infection control programs, specifically recognizing patterns of in-house infections and their potential spread as well as compliance of care staff with all safety programs.
- Ensure proper documentation regarding change in condition and incident reporting for both residents and staff.
- Ensure all paperwork is completed and that proper documentation is maintained in the medical record.
- Coordinate prescription orders with doctors' offices and pharmacy delivery of medication, overseeing central storage and delivery of medications.
- Communicate with residents and responsible parties regarding community's preferred pharmacy and incontinence supply programs.
- Maintain inventory of health-related supplies at all times.
- Plan, schedule, prepare and conduct in-service classes for staff, as needed.
- Respond to resident concerns and complaints in a professional and caring manner.
- Participate in daily "Stand-Up" meetings to communicate key issues within the department.
- Follow and enforce company's safety policies and procedures.
- Must be eighteen (18) years of age.
- Must be a Licensed Vocational or Registered Nurse holding a valid California nursing license in good standing.
- Prefer two (2) years' experience working in Residential Care Facility for the Elderly (RCFE).
- Prefer one (1) year experience supervising and managing employees.
- Residential Care Facility for the Elderly (RCFE) license strongly preferred.
- Able to effectively teach/train staff.
- Able to operate a personal computer and be proficient in MS Office programs (Word, Excel, etc.).
- Able to operate standard office equipment, including fax machine, copier, telephone, etc.
- Knowledge of and/or ability to learn the theory and practice of assisted living and dementia care.
- Able to work with seniors and patiently interact with cognitively impaired individuals.
