Full Time Evening Shift 4pm-12am

Must be able to cover San Fernando Valley/Ventura areas.
Hourly Rate $35.00 - $38.00 + Mileage Reimbursement

This evening shift
position covers the 4pmto 12am shift. During this shift theLVN is available to travel to visit patients at their place of residence and is compensated for the visits made.
Under the direction of a Registered Nurse, the hospice LVN provides skilled nursing and palliative care to patients at their place of residence. The LVN works cooperatively with other Interdisciplinary Team members and provides intermittent visits or shifts of skilled nursing care, including pain management/symptom control, and nursing observations, The LVN provides information about the expected course of the illness, care techniques and other health measures to patients, family members or primary caregivers.
The LVN participates in the care plan development and implementation. Must be able to utilize computers and be comfortable with electronic medical records.
- Graduate of an accredited schoolof vocational nursing.
- Current LVN license within the State of California. Certification in Hospice and Palliative Nursing encouraged.
- At least one year of LVN experience.
- Valid driver's license, automobile insurance, reliable and well maintained transportation.
- Spanish speaking a plus, but not required
