ResponsibilitiesConducts health assessment interviews focused on wound, ostomy, and continence‑related care—performing a focused physical assessment. Collects data and information relative to the health or situation of the resident with wound, ostomy, and/or continence care needs. Analyzes assessment data to determine actual or potential problems/issues related to wound, ostomy, and/or continence care needs. Develops and implements plans for wounds, ostomy, and/or continence care that are focused on strategies to attain expected, measurable outcomes.
Reviews existing orders and makes recommendations to physicians, managers, and other members of the healthcare team that focus on healing, maintaining, or supporting the resident’s wound care goals. Completes weekly wound rounds with wound physician and documents on wounds weekly using Ekare system. When appropriate, teaches, consults, and assists families and residents to assume responsibility for prevention of complications as they relate to patients with wound, ostomy, and continence problems.
Develops, provides, and evaluates staff education programs, orientation, in‑services and continuing education as it relates to wound, ostomy, and continence care. May be assigned other duties as appropriate.
- Graduate of an accredited school of nursing
- Current Missouri nursing license in good standing
- Current CPR/AED certification required
- Wound certification preferred
- Minimum two years of varied work experience as a professional nurse and, preferably, one of those two years in a post‑acute setting
- Must possess critical thinking skills
- Excellent communication skills to interact verbally with patients, visitors, and staff
- Excellent time‑management and organizational skills
- Ability to stand and walk for long periods of time
- Ability to lift heavy objects using proper lifting techniques and with assistance as necessary
- Ability to transfer and move residents who vary with ability to assist weighing 200+lbs. using proper lifting techniques and with assistance as necessary
- Ability to push and pull equipment weighing in excess of 40lbs. using proper lifting techniques and with assistance as necessary
- Manual dexterity, coordination, and skillful use of hands when working with residents and equipment
- Ability to perform necessary documentation
- Visual acuity
- Must be able to manage a diversity of activities
$29.39 - $48.20 per hour
