We are looking to fill multiple vacancies throughout Contra Costa County within the Public Health Division, the Contra Costa Regional Medical Center (CCRMC), and Health Centers.

Community Health Workers work closely with professional staff and perform a variety of tasks associated with the public health program(s) to which they are assigned. Typical tasks include providing information/referral to patients, completing applications, documenting in patient charts, stocking and distributing information/referral materials, arranging transportation for patients being discharged from the hospital, and assisting professional staff with interviews of patients.

The Health Care for the Homeless (HCH) program is part of the Public Health division and provides health services to patients experiencing homelessness in Contra Costa County. The program aims for all people experiencing homelessness to access comprehensive healthcare services to improve health status and manage chronic conditions. The team includes bilingual medical, dental, behavioral health, social support and administrative professionals who provide direct care and referrals.

Detention Health Community Health Workers support the discharge team and participate in CalAIM initiatives, including Medi-Cal applications and facilitating communication among patients, external facilities, and healthcare providers. This position links detention medical services with community resources, ensuring adequate staffing to deliver quality patient care and advocate for health needs aligned with CalAIM. Responsibilities include assisting with Medi-Cal insurance and benefits, collecting data for Medi-Cal and billing forms, and documenting care coordination activities and care plans in the electronic health record.

• Assisting with Medi-Cal and Cal Fresh applications
• Reviewing medical charts for insurance and assisting clients with calls to Medi-Cal and other providers
• Maintaining stock of informational and referral materials

• Arranging transportation for patients and assisting with case management
