• Assess, plan, advocate, and coordinate care from admission to discharge ensuring a safe transition post hospitalization.
• Implement the Care Management model to provide effective transition planning and length‑of‑stay oversight that maintains patient experience, safety, and quality of care.
• Utilize performance metrics and adopt best practices to optimize patient outcomes and operational efficiency.

• Master’s in Social Work (MSW) – required. Awaiting conferment by the educational facility is acceptable.
• Graduate of an accredited School of Nursing – required.

• 0–2 years in an acute‑care hospital setting – required (Social Work role).
• 2–3 years in an acute‑care hospital setting – preferred (Nursing role).
• Care Management or Utilization Management experience – preferred.

• Current license in Social Work in the state of employment (LMSW, LCSW) – required.
• Current license in the state of employment (Nursing) – required.

• Basic Life Support (BLS) – required at hire date or within 90 days of hire.
• Certification in Case Management – preferred.
