Responsibilities
- Perform patient assessments, monitor drug therapy effects using clinical symptoms, laboratory data, point-of-care tests, and diagnostic techniques.
- Attend rounds and medical case conferences to evaluate patient drug therapy regimens.
- Interview patients for drug use, allergies, sensitivities, and medication histories; assess compliance and counsel on appropriate drug use.
- Assist in medication usage evaluation programs, establish criteria for cost effectiveness, control utilization, process indicators, and monitoring parameters; conduct evaluations and summarize results.
- Participate in the establishment of critical pathways to ensure optimal medication usage.
- Provide comprehensive drug information services for healthcare providers and patients, including developing pharmaceutical newsletters, patient education materials, and evaluating formulary requests.
- Design and conduct in-service and educational programs regarding drug therapy for healthcare providers, pharmacists, and students.
- Critically analyze, evaluate, and prepare recommendations of medical literature related to medication therapy and develop treatment protocols or guidelines.
- Interpret and apply pharmacokinetic drug data to recommend patient-specific drug dosage regimens and formulate individualized drug regimens based on patient diagnosis, concurrent disease states, pharmacokinetics, pharmacoeconomics, and patient condition.
- Advise and assist in implementing cost containment strategies for pharmaceutical product usage.
- Review journals and periodicals, meet with pharmaceutical product specialists to discuss market conditions, managed care implications, disease state management, and obtain information to implement cost containment strategies and develop therapeutic outcome studies.
- Participate in quality improvement, quality assurance, and performance improvement activities to ensure optimal pharmaceutical care.
- Perform root-cause analysis for medication errors and develop prevention plans using EHR system, technologies, medication behaviors, and workflow improvements.
- Review, analyze, synthesize, present, and publish pharmacy formulary decisions and information.
- Generate and analyze clinical dashboards and screen appropriate patients for clinical pharmacist referrals.
- Derive and manage medication treatment therapy for patients through prescribing, modifying, and discontinuing medications.
- Provide pharmacy informatics and technology input and assist with pharmacy or medication process system testing.
- Dispense medication as necessary.
- Perform inspections of medication storage rooms or areas.
- Option I:
One year of specialized clinical pharmacy experience. - Option II:
Successful completion of a pharmacy residency program in clinical pharmacy. - Current license to practice as a Registered Pharmacist issued by the California State Board of Pharmacy.
- Valid California Class C Driver License or an alternative method of transportation to carry out job-related essential functions.
Out-of-class experience will not be accepted for this examination. Required experience must be fully met and indicated on the application by the last day of filing.
Physical Class III – ModerateStanding or walking most of the time with bending, stooping, squatting, twisting, and reaching; working on irregular surfaces; occasionally lifting objects weighing over 25 pounds and frequent lifting of 10-25 pounds.
