Responsibilities

Powerback delivers recovery, rehabilitation, respiratory, and wellness services to patients across the nation through an empowered, patient‑centered approach. The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population, either in a private residence or an independent living facility. The role includes evaluating, developing, and delivering individualized physical therapy programs to facilitate rehabilitation, making referrals to other disciplines, instructing and training patients and caregivers, and maintaining a flexible schedule in various settings.

  • Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
  • Ensure adherence to department and patient treatment schedules, timely clinical and billing documentation, and participation in work‑related meetings.
  • Attend and contribute to patient care, staffing conferences, and other related meetings.
  • Make referrals to other disciplines and agencies to address comprehensive patient needs.
  • Collaborate with the rehabilitation team to select the most appropriate therapeutic regimen consistent with each patient’s needs and capabilities.
  • Adapt program and/or plan of care according to the individual patient’s needs.
  • Promote a safe environment and enforce the elimination of fire and safety hazards.
  • Order supplies and equipment as necessary.
  • Maintain and report any equipment repairs needed.
  • Prioritize patient service: ensure that patients and families receive the highest quality service in a caring, compassionate atmosphere that respects individual needs and rights.
  • Screen, examine, and evaluate patients, including history, systems review, and appropriate testing, to establish a diagnosis, identify impairments, and design a plan of care.
  • Develop treatment goals and methods, collaborate with patient and caregivers, implement the physical therapy treatment plan, and document all services.
  • Provide services such as therapeutic exercise, functional training, manual therapy, airway clearance techniques, integumentary repair and protection, electrotherapeutic and mechanical modalities, health and wellness programs, chronic disease management education, fall risk assessment and interventions, and community case management.
  • Design, fabricate, fit, and/or apply adaptive devices, training patients in their use.
  • Select activities suited to the patient’s physical, mental, and cognitive capacity.
