This Primary Care Physician will serve as a clinical leader and direct care provider within this PACE program. You’ll manage the ongoing care of older adults through an interdisciplinary team model — balancing autonomy, team collaboration, and participant-centered care.
You’re not just treating patients — you’re leading the IDT (Interdisciplinary Team), influencing outcomes, and helping seniors maintain independence in their communities. Key Responsibilities- Conduct comprehensive initial, semi-annual, and annual assessments for PACE participants.
- Lead and collaborate with the interdisciplinary team to develop and maintain individualized care plans.
- Provide primary and ongoing medical care to assigned participants — both in-clinic and in nursing home settings as needed.
- Supervise and collaborate with Nurse Practitioners and Physician Assistants.
- Manage hospital and emergency coordination, including discharge planning and follow-up.
- Provide telephonic after-hours coverage on a rotating schedule.
- Participate in quality improvement (QAPI) initiatives and clinical meetings.
- Support their mission to promote autonomy, dignity, and quality of life for frail and elderly adults.
- Education: M.D. or D.O. with active California medical license.
- Training: Board Certified or Board Eligible in Family Medicine or Internal Medicine;
Geriatrics training preferred. - Experience:
- Minimum 1 year working with frail or elderly populations (PACE, SNF, Geriatrics clinic, or similar).
- Demonstrated ability to thrive in a team-based care model.
- Certifications:
- DEA certificate
- Basic Life Support (BLS)
- Skills:
- Excellent communication and collaboration with multidisciplinary teams.
- Leadership ability with NPs/PAs.
- Strong EMR documentation and follow-through.
