  • Job Title: YES Waiver Coordinator – Actively pursuing licensure
Job Summary
  • Provide treatment through Texas Resiliency and Recovery for children and adolescents enrolled in mental health services with The Gulf Coast Center (GCC).
  • Implement the YES Waiver Program, primarily through targeted case management, service coordination, transition planning, and crisis/safety planning.
  • Utilize the National Wraparound Initiative model for intensive case management and be trained as a Wraparound Facilitator.
  • Assist individuals in obtaining YES Waiver Medicaid and facilitate development of natural supports, assess treatment needs, and develop plan of care.
  • Coordinate waiver services, assess progress, create crisis/safety plans, and assist with transition planning.
  • Maintain a caseload as determined by the Program Manager and work closely with comprehensive waiver providers and HHSC.
  • Lead a team, collaborate with others, maintain flexibility, and implement TRR services as part of an interdisciplinary team.
  • Ensure proper documentation in the electronic health record and complete required consents and documents.
  • Provide training, psychiatric assessment, crisis management, and coordination of services for assigned caseloads.
Job Duties
  • Manage assigned caseload as part of an interdisciplinary team following program procedures.
  • Conduct Children's Need & Strengths Assessment every three months; complete CANS recertification.
  • Display knowledge of TRR protocols during intakes.
  • Ensure all necessary consent forms are completed and updated annually.
  • Complete QMHP assessment and all required Center forms accurately.
  • Conduct adequate assessment of consumer history before services.
  • Maintain complete documentation required for TRR and Center policy.
  • Establish initial person‑centered recovery plan with SMART goals/ objectives.
  • Review services and document progress; discuss next treatment period with youth and LAR present.
  • Document service details to demonstrate medical necessity.
  • Adhere to Voice and Choice Philosophy of the Wraparound Approach.
  • Monitor service frequency to meet Level of Care requirements.
  • Utilize Smart Care reports to monitor documentation.
  • Complete all documentation within two business days.
  • Provide services per Youth Behavioral Health Service Standards and ensure valid CANS.
  • Arrange referrals, discharge, or transfer of services; complete summaries and fair hearing letters.
  • Participate in team meetings and maintain professional demeanor.
  • Use mature problem‑solving approach in working with consumers and colleagues.
  • Maintain contact regarding phone and electronic messages; report flextime to supervisor.
  • Apply ‘Fish’ philosophy in assigned work duties.
  • Meet performance requirements for entire caseload each month.
  • Participate in the Administrative Claiming process.
  • Promote agency compliance policy and support community partnerships.
  • Provide courteous, respectful service to all individuals.
  • Report personnel leave, flex time, and emergency leave to supervisor.
  • Follow GCC's Business Code of Conduct, Corporate Compliance Plan, Dress Code, and Safety guidelines.
  • May travel within Brazoria and Galveston Counties; out‑of‑region travel for training.
  • Maintain a clean driving record and Texas driver’s license with liability insurance.
  • Require current Texas Driver's License, reliable transportation, and auto liability insurance.

  • Receives direct supervision from Program Manager‑YBHS.

  • Master’s degree in behavioral sciences field from an accredited institution.
