Case Manager Registered Nurse (RN)
Status/FTE: FT / 0.8-1.0 FTE
Shift length: 8 hours
Start Time: 0800
Unit: Care Management
Schedule Requirements (including weekends, holidays): Monday-Friday, rotating weekends.
Required Qualifications- Current state RN license
- Associate’s degree from an accredited nursing school
- BLS for Healthcare Provider upon hire
- Three (3) years of acute care experience
- Must have strong knowledge of managed care, medical assistance, and Medicare reimbursement.
- Working knowledge of advanced directives, guardianship, abuse and neglect.
- BSN preferred
- One (1) year of case management experience
The role of the Hospital RN Case Manager is to increase patient throughput to the most appropriate level of care while facilitating interdisciplinary care across the continuum. The RN CM will assist in identifying patients appropriate for admission, observation, or other patient status. They will function as a liaison between physicians, physician advisors, other professional staff, residential or nursing facilities, community agencies, patients and families, and department staff.
The role includes the management of social issues that impact patient classification and ongoing medical management of patient care needs. The RN CM will assist in identifying physician and staff documentation opportunities to support quality and pay for performance indicators. Utilization review on authorization of observation, inpatient, and surgical patients may be required at times in this role to assist with claims and denials.
Goal is to collaborate with patients, families, and healthcare providers to promote health and manage illness in a cost effective and quality driven manner.
