Director of Case Management

Position Overview:
We are seeking an experienced Director of Case Management to oversee care facilitation, patient flow, and hospital continuum management
. This role is responsible for leading Case Management, Social Work, Utilization Management, and Post-Acute Services Coordination to ensure seamless patient transitions and optimal care progression.

• Develop and implement a hospital-wide Case Management program to enhance patient care coordination.
• Oversee staff roles, care progression, and physician collaboration to improve patient outcomes.
• Ensure compliance with utilization management, social work, and post-acute services processes.
• Drive strategic initiatives to enhance patient flow, discharge planning, and care transitions
  .

Required:

• Active state RN license
  .
• 5+ years of case management experience in a healthcare setting.
• Proven leadership, communication, and customer relations skills
  .

Preferred:

• 3-5 years of supervisory experience
  .
• Formal case management training
  .
