What is a Patient Care Coordinator?- An entry-level office role that is responsible for maintaining pleasant and consistent daily operations of the clinic.

- Patient Care Coordinators learn new things – a lot! They multitask in multiple computer programs each day.
- Greets everyone who enters the clinic in a friendly and welcoming manner.
- Schedules new referrals received by fax or by telephone from patients and physician offices.
- Verifies insurance coverage for patients.
- Collects patient payments.
- Maintains an orderly and organized front office workspace.
- Other duties as assigned.

The Patient Care Coordinator - I (PCC-I) supports clinic growth through excellence in execution of the practice management role and patient intake processes. This individual will work in collaboration with the Clinic Director to carry out efficient clinic procedures. The PCC-I position is responsible for supporting the mission, vision, and values of Upstream Rehabilitation.
Responsibilities- Core responsibilities
- Collect all money due at the time of service
- Convert referrals into evaluations
- Schedule patient visits
- Customer Service
- Create an inviting clinic atmosphere.
- Make all welcome calls
- Monitor and influence arrival rate through creation of a great customer experience
- Practice Management
- Manage schedule efficiently
- Manage document routing
- Manage personal overtime
- Manage non-clinical documentation
- Manage deposits
- Manage caseload, D/C candidate, progress note, and insurance reporting
- Monitor clinic inventory
- Training
- Attend any required training with the Territory Field Trainers for Raintree and other business process updates.
- Complete quarterly compliance training.
- High School Diploma or equivalent
- Communication skills – must be able to relate well to Business Office and Field leadership
- Ability to multitask, attention to detail, ability to meet deadlines, work with little to no supervision
- As a member of a team, must possess efficient time management and presentation skills
- Inside environmental conditions; protections from weather but not necessarily from temperature changes; exposure to noise typical of an indoor environment.
- Full-time position with normal business hours Monday through Friday, minimum 40 hours per week; may require some evenings and weekends for events or as needed.
- sedentary work; constant sitting with ability to alternate with standing as needed.
- Constantly communicates with associates; must be able to hear and speak to exchange information.
- Frequently operates a computer and office equipment.
- Must be able to read information on a computer screen and comprehend provided information.
- Occasionally lifts 0-20 lbs.; carries, pushes, and pulls 0-20 lbs.
- Rarely crawls, crouches, kneels, stoops, climbs stairs or ladders.
