The SSBC Program Site Lead & Coach oversees the effective delivery of the Small Steps for Big Changes diabetes prevention program at the YMCA of Greater Toronto, ensuring alignment with organizational standards and evidence-based practices. The role provides site-level leadership and coordination to support consistent program implementation, smooth operations, and a positive, participant-centered environment. In addition, the position delivers individualized coaching to support individuals at risk of type 2 diabetes in adopting sustainable lifestyle changes related to physical activity and nutrition.

Through a balance of program coordination and direct service delivery, the role contributes to improved health outcomes and advances the YMCA’s commitment to community well-being.

In this role, you will:

  • Liaise with the Regional Research Coordinator to plan Small Steps Big Changes (SSBC) program delivery, including participation in a virtual Adaptation and Implementation workshop.
  • Meet regularly with the YMCA Senior Director of Programs and the Regional Research Coordinator to share feedback, insights, and observations related to program delivery.
  • Oversee SSBC program delivery by coordinating site-level activities, supporting coaches through the training process, and conducting regular check-ins to ensure consistency and quality.
  • Assist the SSBC team with local recruitment efforts to support participant enrollment and program reach.
  • Conduct initial assessments to evaluate participants’ lifestyle habits, fitness levels, and dietary patterns.
  • Develop individualized action plans that outline tailored exercise and nutrition approaches aligned with each participant’s needs, goals, and preferences.
  • Provide one-on-one coaching sessions focused on exercise and nutrition to support participants in achieving their personal goals.
  • Apply motivational interviewing techniques to support goal setting, address barriers, and build participant confidence in making sustainable changes.
  • Monitor participant progress by tracking goal adherence, providing feedback, and adjusting plans as needed.
  • Encourage self-monitoring by teaching participants to use fitness trackers, food logs, or digital applications to track progress.
  • Promote sustainable lifestyle changes by reinforcing healthy habits that can be maintained beyond the coaching period.
  • Provide ongoing support and encouragement to maintain participant engagement and commitment throughout the program.
  • Ensure a safe training environment by maintaining first aid certification and responding appropriately to health concerns during physical activity sessions.
  • Maintain accurate and up-to-date documentation of client appointments using the online onboarding log, scheduling forms, and coach checklists.
  • Perform other duties as assigned.
  • You bring:

  • Successful completion of the Small Steps Big Changes (SSBC) Coach Training Program (approximately 10–12 hours) is required.
  • Current Emergency First Aid Level A certification is considered an asset.
  • Background or knowledge in Health Sciences is considered an asset.
  • Ability to use digital tools to track participant progress, communicate effectively, and document key information.
  • Strong leadership, communication, and organizational skills.
  • Well-developed interpersonal and relationship-building skills, with the ability to establish rapport and communicate effectively with members, staff, and volunteers.
  • Ability to encourage and motivate members and volunteers, while acknowledging and appreciating achievements and contributions.
  • Ability to work flexible hours, including days, evenings, and weekends.
