E-MDT (Elder Abuse Enhanced Multidisciplinary Coordinator)

POSITION

DESCRIPTION:

BRIDGES operates a variety of programs guided by the Philosophy of Independent Living. The E-MDT Coordinator is essential to providing coordination of services for the Enhanced Multi-Disciplinary Team addressing elder abuse in the lower Hudson Valley of adults aged 60 and older. The E-MDT approach, delivered locally, provides improved and effective cross-system collaboration and specialized responses to elder abuse of older adults aged 60 and older.

This includes responses for financial exploitation, physical abuse, emotional abuse, sexual abuse, and neglect by others.

QUALIFICATIONS General
  • A Bachelor’s Degree (Master’s Degree preferred) and/or equivalent work life experience in a similar capacity with the population served.
  • Excellent interpersonal, organizational, written and oral communication skills required; able to interact in a respectful, supportive and confidential manner
  • Strong familiarity with Independent Living Philosophy, support for the senior and disability community
  • Remain current in all agency required trainings
  • Maintain strong work ethics, and strict adherence to code of ethics, standards of conduct and confidentiality requirements
  • Self-motivator and ability to work independently
  • Capable of communicating in a manner that is culturally sensitive to individuals with disabilities
  • Must be able to travel locally for meetings, home visits or events
  • Working knowledge of Microsoft Office Software, including Excel, Word and Power Point.
  • Working knowledge of Gmail/Google applications
  • Ability to work independently in an efficient manner

DUTIES & RESPONSIBILIITIES General
  • Utilizes NYS Office for the Aging policies provided by Lifespan and develops procedures specific to the Lower Hudson Valley region
  • Reach out to local professional organizations, developing partnerships and collaborations, and engages key stakeholders
  • Determines and creates regular meeting schedule for E-MDT members
  • Coordinates and facilitates E-MDT meetings

The mission of BRIDGES is to provide advocacy and leadership toward an accessible, integrated community, promoting health and autonomy for people with disabilities.

  • Finds, identifies, and vets elder abuse case referrals for presentation at E-MDT meetings
  • Coordinates action plans, case follow-ups and responses to case issues emerging between meeting dates in a timely manner
  • Keeps accurate records for E-MDT meetings and case consultations
  • Contracts with a civil legal service provider for access to services within the region
  • Identifies and establishes a working relationship with a mental health provider for access to services as needed
  • Establishes and continues a working relationship with a forensic accountant for access to services as needed
  • Collects and enters required data into the mandated data system
  • Completes team effectiveness survey
  • Participates in monthly technical assistance sessions
  • Participates in monthly E-MDT Coordinator Peer Leadership Group
  • Completes daily activity log using OVS Functional Time Record
  • Maintains strict confidentiality and protects Agency operations by keeping information confidential and secured
  • Adheres to all Agency policies and procedures, including but not limited to corporate compliance, personnel and fiscal policies and procedures
  • Provides monthly reporting as required
  • Offer support and guidance for clients to maintain their best quality of life
  • Identify barriers (internal and external) to full participation in community resources and developing strategies to overcome those barriers.
  • Maintain accurate record keeping, note taking, and data entry
