**** Patient Care & Coordination
*** Deliver compassionate, skilled patient care under RN supervision.
* Perform assessments, procedures, and interventions based on physician orders.
* Collaborate with RNs, therapists, and care teams to ensure seamless care.
* Be the eyes and ears for changes in patient condition, helping adjust care plans in real time.
* Stay focused and efficient while keeping patients at the heart of everything you do.
** Patient & Caregiver Education
*** Educate patients and families on self-care practices and safety.
* Be initiative-taking in identifying and reporting any safety concerns.
** Communication & Documentation
*** Keep documentation accurate, timely, and compliant.
* Jump into care team huddles, share insights, and keep communication flowing.
** Professional Growth & Team Support
*** Stay sharp through ongoing training, in-services, and continuing education.

** Requirements*
* ** You’ll Bring to the Table:
**** Education**
* ** High School Diploma or equivalent –
* Required.
***** Licensure**
* ** Current LPN license in the state of Georgia –
* Required
* **** Experience**
* ** At least 3+ years of recent medical-surgical or acute care nursing experience –
* Required.
***** Certifications**
* ** Basic Life Support (BLS) from the American Heart Association –
* Required
